Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that are likely to cause burns, serious injury or death in that employees are exposed to fire and explosion hazards. a) Two cyclones, with a total volume of 487 cubic feet each, separated polyurethane dust from the incoming air stream which was pulled by two 60hp fans to remove dust from the recycle storage bins. The dust that was suspended in and moving through the cyclones was combustible with a Kst of 16.72 bar/m*s. The cyclones were located inside the main building near the grinding equipment in an area where employees worked on a daily basis. No deflagration panels, vented to the outside, or other explosion protection was installed on the cyclones. Employees working in the area were in danger of fire or explosion hazards associated with the combustible dust being handled by the cyclone. b) Two 487 cubic feet cyclones vented exhaust air to the building and there was no filter to capture dust in the exhaust stream or means to prevent transmission of flame and/or explosion energy from the exhaust duct to the building. Employees working in the area were at risk of being burned from the flame front coming out of the recycle duct. The dust being exhausted to the building in the air stream had a Kst of 16.72 bar/m*s. ABATEMENT NOTE: Among other methods, one feasible and acceptable method to correct these hazards is to comply with the National Fire Protection Agency NFPA 654 "Standard for the Prevention of Fire and Dust Explosions from the Manufacturing, Processing, and Handling of Combustible Particulate Solids 2006 Edition" NFPA 68 "Standard on Explosion Protection by Deflagration Venting" and NFPA 69 "Standard on Explosion Prevention Systems 2008 edition"; including but not limited to: (a) Install explosion relief venting on all cyclones venting to an outside area (no more than 20 ft. straight duct) with no potential to be occupied by employees or install a suppression system in all cyclones (b) Install filters for the exhaust from the cyclonic dust collector/separator to ensure that filtering efficiency is 99.9% efficient at 10 microns.
