Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by falling equipment. Location: World trade center- Vehicle Inspection Center On or about 7 May, 2011 Drilling adapters were placed on the top of a drilling rig while the rig was being repositioned. A worker was struck by one of the adapters when it fell off the rig as he was moving/operating the rig. Abatement Note: Among other methods, one feasible and acceptable method to correct this hazard is to comply with the rules from the Casagrande equipment operators manual, Volume 1 (safety information), paragraph 1.2.2.: The operator must make sure that the machine and the area around the machine is kept free of all moveable objects that are not part of the machine (tools left maintenance work, ropes used for lifting part of the machine, personal possessions, etc). NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.
