Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. Employees were exposed to the hazard of being crushed by a moving hoist car while working underneath the car. a)South side of building: Employees were exposed to being caught and crushed between the bottom of the hoist car and the hoist support column while removing bottom terminal limits during the dismantling of the exterior hoist system. Safety procedures were not taken. On or about 04/20/11. Among other methods, one feasible and acceptable method to correct this hazard is: Comply with the American National Standard A10.44-2006 Control of Energy Sources (Lockout/Tagout) for Construction and Demolition Operations, and while working underneath the car. a)South side of building: Employees were exposed to being caught and crushed between the bottom of the hoist car and the hoist support column while removing bottom terminal limits during the dismantling of the exterior hoist system. Among other methods, one feasible and acceptable method to correct this hazard is: Comply with the American National Standard A10.44-2006 Control of Energy Sources (Lockout/Tagout) for Construction and Demolition Operations, and establish and enforce a program consisting of energy control procedures, employee training, and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. 1st REPEAT All-Safe, LLC.
