Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire, high pressure release of a pressurized vessel: a. On or about February 10, 2012, the employer did not implement procedures to protect employees from the hazards of a high pressure hyperbaric oxygen therapy (HBOT) chamber, to include the prevention of fire inside the chamber and violent over pressurization/rupture of the HBOT chamber. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the requirements set forth in NFPA 99-2012, Health Care Facilities, Chapter 14 - Hyperbaric Facilities; the HBOT chamber manufacturer's operations, maintenance and training manuals; and the safety relief valve manufacturer's technical reference.
