Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing, or likely to cause, death or serious physical harm to employees in that employees were exposed to crushing hazards: On or about May 10, 2011, and at other times prior thereto, at the Hope Schorlemer 01-01H well site, employees were not protected against crushing hazards during work truck positioning activities. Among others, one feasible means of abatement includes: a.completing a job safety analysis (JSA) in accordance with the employer's written safety and health program and the International Association of Drilling Contractors (IADC), "Accident Prevention Reference Guide", Section 1.4 b.assigning a spotter with unobstructed aerial view and no other job responsibilities in accordance with the employer's written safety and health program and the International Association of Drilling Contractors (IADC), "Accident Prevention Reference Guide", Section 7.0 - GENERAL RULES, Item (K).
