Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the potential hazards of unexpected energization or start up of machines or equipment or release of stored energy: On or about 04/27/2011, at 4200 N. Flager Drive, West Palm Beach, FL 33407, employees were working on the Transom deck of a Yacht preparing the Laserette door for painting. Deficiencies in the Safety and Health Program were evident in that employees preparing a yacht for painting were not provided with task specific Lock-out/Tag-out procedures, thus exposing workers to the hazard of being crushed by a hydraulically operated Laserette door on the Transom deck. Among other methods, one feasable and acceptable abatement method to correct this hazard is to develop specific Lock-out/Tag-out procedures for equipment such as, but not limited to, doors, hatches, and ladders to ensure the control of hazardous energy; provide training and procedures for effective energy contol to employees; and conduct a periodic inspection of the energy control procedures at least annually.
