Section 5(a)(1) of Public Law 91-596, the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to: Hazards of flying shrapnel from exploding plastic PVC pipe. (a)Site wide - On or about February 29, 2012 employees were exposed to the hazards of flying shrapnel from an exploding plastic PVC pipe, Genova 1120 Schedule 10 PVC pipe used to transport compressed air throughout the machine shop, fabrication shop, warehouse and the water jet area. The compressed air was at an operating pressure of 80 pounds per square inch (PSI). Among other methods, the following are feasible and acceptable methods of correction. 1. Replace the current PVC compressed air piping system with materials suitable for the maximum air pressure as recommended by the Plastic Pipe Institute, such as metal. 2. Follow the American National Standards Institute/American Society of Mechanical Engineers (ANSI/ASME) B 31.1-2007, Power Piping, as applicable.
