Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to the employees in that employees were exposed to the hazard of impact and/or crushing injuries associated with the rollover of a lift truck without the use of an operator restraint system: (a)Entrance drive to facility: An employee was operating a Terex Rough Terrain Forklift, Model SS-842 without utilizing the seat belt. The employee lost control of the vehicle and ran off the road, the vehicle turned up on its side and the operator suffered a fatal head injury, as determined on August 2, 2011. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard would be to follow; ASME B56.6-2002 Safety Standard for Rough Terrain Forklift Trucks and require the usage of an operator restraint system at all times and to follow the manufactures operating manual and wear the provided seat belts at all times when operating the lift trucks.
