Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not furnish employment and a place of employment which are free from recognized hazards that was causing, or likely to cause, death or serious physical harm to employees in that employees are exposed to struck-by hazards: This violation occurred on November 27, 2010, where each employee working on the rig floor during the installation of the top drive track/guide beam were not protected from the unsecured track/guide beam as it was raised up over the rig floor in the event of a hook pin failure. Among other methods, one feasible and acceptable method to correct his hazard would be to: a.Perform monthly inspection of hook pin welds for cracks and yearly (or after approximately 3,000 operating hours) Nondestructive Examination which includes a Magnetic Particle Inspection on guide beam joints as recommended in the manufactures service manual: National Oilwell Varco, TDS-11SA,Top Drive System Service Manual, SM00856, Revision B, Section 5, page 3. b.Follow the American Petroleum Institute Recommended Practice 8B, Seventh Edition, March 2002, ANSI/API RP8B/ISO 13534-2000 for Inspections, Maintenance, Repair and Remanufacture of Hoisting Equipment; Section 5, Inspection and Maintenance. Pursuant to 20 CFR 1903.19, within ten (10) calendar days of the abatement date of this citation, the employer must submit an abatement plan showing it is in compliance with the standard, including the steps it is taking to ensure the track/guide beams are inspected, to prevent failure during installation and the hook pin on all track/guide beams are regularly inspected for cracks and defects and subsequently reported and repaired.
