Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck-by the unexpected disconnection of tow hooks from the wire mail containers. On or about 10/25/2011 near the flats unit, between columns J5 and H5, an old model of wire container that was being towed by an employee became disconnected from the Taylor-Dunn Tow Tractor(Model E) and resulted in a partial finger amputation to the employee. One method of abatement to correct this apparent hazard is to push or fork the old model wire containers such they are not equipped with the proper welds to be safety connected to the Tow Tractor. ANSI B 56.8 7.3.3.2 requires that operator only handle stable and safely arranged loads. Another method of abatement to correct the hazard is to replace the older model wire containers with new ones that are equipped with the appropriate connection points. Abatement certification and abatement documentation is required for this violation.
