Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by, crushed-by and fall injuries from the special purpose personnel elevator: a)Employees working on the ground floor level were exposed to struck-by and crushing injuries in that the landing opening for the special purpose personnel elevator was not adequately protected by a barricade preventing entry into the landing area when the elevator car is traveling downward. b) Employees working at the bin deck level were exposed to struck-by and fall injuries in that the front and back sides of the landing opening for the special purpose personnel elevator were not adequately protected by barricades preventing entry into the landing area when the elevator car is not at the bin deck level. Among other methods, one feasible and acceptable method to correct this hazard is: Enclose the landing area with a 7 foot high door with the controls for the manlift outside of the enclosed area in accordance with ASME A17.1-1996, American Society of Mechanical Engineers, in Safety Code for Elevators and Escalators, Rule 1500.6a Hoistway Doors and Gates, which requires that the full width of each landing opening be protected to its full height, or 7 feet, whichever is less, by doors or gates. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is completed.
