Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of struck-by and crushed-by falling objects from pull/drag loads: This violation was observed on or about September 27, 2010, at the 11129 E. 56th St., S., Tulsa, OK worksite, a Toyota forklift was chained to a Sciaky resistance welder and used to horizontally pull/drag the welder to the warehouse door. Among other methods, one feasible and acceptable method of abatement would be to ensure forklift operations are carried out in accordance with the practices set forth by the equipment manufacturer and or ANSI B56.1-1969 American National Standard for Powered Industrial Trucks. Pursuant to 29 C.F.R. 1903.19 within ten (10) calendar days of the abatement date, the employer must submit documentation describing actions it is taking to establish, communicate, implement written procedures to ensure that machine/equipment is moved safely and within the specifications of the equipment moving the load.
