Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by an inflatable pipe plug: Location: Cropsey Avenue and 37th Street Brooklyn NY,11214. On or about 09/30/10. a) A Petersen Inflatable pipe plug exploded while the employees were trying to locate a leak in a 42" diameter Sewage pipe. Abatement Note: Among other methods abatement of this hazard can be achieved by reading and following the manufacturer's safety instructions for the proper use of pneumatic pipe plugs. -Before and after each use, clean the plug and inspect for surface tears, cuts or any other damage -Stay clear of any part of the plug when it is inflated. -Use a long enough hose to keep the inflation gauge(s) and valve(s) a safe distance from the plug. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19
