North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to extended exposure to anhydrous ammonia due to improperly maintained/fitted doors where the broken doors allowed emergency ventilation of the atmosphere in the engine rooms to be reduced: a)House of Raeford (Slaughter Facility), Engine Room #4- Where the left side door of the wood-framed fiberboard double door at the southeast end of engine room#4 was hanging from a single hinge and would not close properly and was not self-closing and tight fitting. b)House of Raeford (Slaughter Facility), Engine Room #4 - Where the right side door of the wood-framed fiberboard double door at the southeast end of engine room#4 would not close properly and was not self-closing and tight fitting. Abatement note: Among others, one reasonable and feasible method of abatement would be to repair the doors to allow full closure and fitting. Reference IIAR Bulletin 112, paragraph 4.2.1(b). "... Exit doors shall swing outward, be equipped with panic-type hardware, and shall not be locked while machinery room is occupied. Doors shall be tight-fitting, and self-closing."
