Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an amputation hazard. a) On or about 6-17-10 in building #3995, no protective cover was provided on the pedal mechanism attached to the Power Press Brake machine exposing employees to crush-by and amputation hazard. Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide a guard or cover for the foot pedal that meets the requirements on ANSI B11.3-2002 section 6.10.4 Safety Requirement for Power Press Brakes. Which states: When provided, foot controls shall be protected to prevent actuation from falling or moving objects, or from unintended activation. 1) You are not limited to the abatement methods suggested above. 2) The methods explained are general and may not be effective in all cases. 3) The employer is responsible for selecting and carrying out an appropriate method.iate
