Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards: (a)Road Widening Project Arc Light Blvd and Marianas Blvd - On or about 7 JUN 10 and at times prior thereto, the Komatsu Excavator (Model # PC200-8N1/Serial # 310304) had the bucket attached to the excavator arm and was secured by a bolt with a single nut on one side. AMOUNG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHODS TO CORRECT THE HAZARD: 1.To follow the American Equipment Manufacturers (AEM) "Safety Manual for Operating and Maintenance Personnel" which requires that damaged and missing parts are replaced, and 2.To follow the Manufacturer's Operator Manual by using only service parts meeting manufacturer specification, and 3.To train operators and maintenance personnel to inspect equipment for defects and to complete service and repairs before equipment is placed back in service.
