Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards due to being struck by collapsing industrial steel storage racks. a) Worksite, 340 Hendrickson Avenue, Lynbrook, NY 11204 - On 09/27/10 employees were exposed to crushing and fall hazards while assembling industrial steel storage racks. The racks collapsed injuring the employees. ABATEMENT NOTES: Among other methods, feasible method's of abatement to reduce this hazard is to: a. Follow manufacturers assembly instructions and drawings, (Ref. American National Standard Institute, M H 16.2.1984. Manual of Safety Practices - A Code of Safety Practices for the Use of Industrial and Commercial Steel Storage Racks, Page 5, section 1.2.) b. All columns of racks are to be furnished with bearing plates, (Ref. American National Standard Institute, M H 16.2.1984. Manual of Safety Practices - A Code of Safety Practices for the Use of Industrial and Commercial Steel Storage Racks, Page 5, section 1.4.) and (ANSI MH16.1 -2004, page 3 1.4.7) c. Diagonal bracing configurations can only be determined by reference to the engineering specifications, (Ref. American National Standard Institute, M H 16.2.1984. Manual of Safety Practices - A Code of Safety Practices for the Use of Industrial and Commercial Steel Storage Rack Page 5, section 1.5c.) d. Instruct the employees regarding the necessary precautions to observe when moving the racks, particularly the associated hazard of any applied force in the direction of the rack's depth direction. Instruct employees not to climb on racks. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
