Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious harm to employees. Employees were exposed to struck by and crushed by hazards: a) Workplace- The employer workplace practice is to leave keys in the ignition and in the "on" position while the vehicles are unattended. This creates employee exposure to the unexpected movement of these carts; On or about 09/27/10. ABATEMENT NOTES: Among other methods, several feasible methods to reduce this hazard are to: 1) Follow ANSI/ITSDF Safety Standard for Personnel and Burden Carriers (B.56.8-2006.) Specifically: 7.3.1.5- When a carrier is to be left unattended, stop the carrier, apply the parking brake, stop the engine or turn off the power, turn off the control or ignition circuit and remove the key. Block the wheels if carrier is left on an incline. Additionally, for electric carriers, the forward and reverse directional controls should be neutralized. 7.3.1.6- A carrier is considered unattended when the operator is 25 feet or more from the carrier which remains in his view, or whenever the operator leaves the carrier and it is not in his view. When the operator is dismounted and within 25 feet of the carrier still in his view, he still must have controls neutralized and parking brake set to prevent movement. 7.2.1- The user shall conduct an operators' training program prior to use of cart. 7.2.2- Succesful completion of the operators' training program shall be required prior to operation of the carrier. The program shall be conducted in its entirety to all new operators and not condensed to those claiming prior experience. 2. Follow manufacturers' operating instructions (which are posted on the dashboard of each golfcart.) They include, but are not limited to; Before leaving the vehicle, turn key "off", place "Forward/Reverse" switch in Neutral and engage the PARK (upper) brake. Note: The employer is required to submit abatement documentation for this item in accordance with 1903.19
