Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries while working in close proximity to a wheel loader: a)Yard Behind Shredder: An employee was operating a Mega 400 V wheel loader with an inoperable audible travel alarm, on or about 12/15/2011. ABATEMENT NOTE: One feasible and acceptable method to correct this hazard is to follow the Doosan Wheel Loader Operation & Maintenance Manual Movement Alarm section (page 1- 27) which states that if a wheel loader is equipped with an audible travel movement alarm, test alarm on a daily basis. Audible alarm should sound as soon as travel system is engaged.
