Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: a) On or about September 8, 2010, throughout the facility. The platform used to elevate employees approximately 25 feet high to work on pipes, lights and other maintenance, was not equipped with means to securely attach to the forks and no anchor point to attach lanyards. The load capacity of the platform was unknown. Employees could sustain injuries such as crushed bone, and internal injuries resulting in permanent disability or death.
