29 CFR 1910.106(h)(4)(iv)(a): Equipment was not designed and arranged to prevent the unintentional escape of liquids and vapors and to minimize the quantity escaping in the event of accidental release: a.In the process area, the employer did not ensure that vessel flanges and/or fittings such as, but not necessarily limited to, V-301 were re-torqued prior to the process start up thereby exposing employees to a fire hazard. On or about July 17, 2010, a fire occurred in the process area that was the result of a flange and/or fitting on or in the vicinity of vessel 301 that failed to contain flammable liquids which leaked to an ignition source.
