Text For Citation: 01 Item/Group: 001 Hazard: HEAT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment, and a place of employment, which were free from recognized hazards that were causing, or likely to cause, death or serious physical harm to employees in that employees were exposed to the hazards of a BLEVE: On or about June 21, 2010, in the heavy equipment repair/welding area, an employee was tasked with removing a bolt with a sheared-off head from the track of a Caterpillar D6R Dozer. The employee sprayed the broken bolt with a penetrant and after approximately 48 hours applied an oxygen/acetylene torch to the side of the bolt well. The heat from the torch created a build up of pressure from expanding vapors, which forcefully expelled the headless bolt from the well/cavity, striking the employee. Among other methods, feasible and acceptable means to abate this hazard would be, such as, but not limited to, the following: Contract the project to a third party company with the means to perform the task.
