Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a) 1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards due the use of improper rigging equipment during hoisting operations: a)Under the Hoist Table on the east side of the Planer Mill: On or about 18 May 2010 and at times prior to; the employer failed to protect employees from struck-by and crush hazards while using a "job-made" chain sling and D-ring connector that was not tested, rated or approved for overhead hoisting operations. Among other methods, one feasible and acceptable method to abate this hazard is to: Purchase and utilize, approved and properly rated, alloy steel grade chain slings and attachments for all overhead lifting operations. If job-made slings are to be used, ensure the chain, rings, attachments and other components are approved for overhead lifting operations. Prior to placing job made slings into service have the slings proof tested and maintain the certification in accordance with recognized and accepted good engineering practices.
