Text For Citation: 01 Item/Group: 001 Hazard: CONFINED
North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to flash fire burns: a)job site - on or about February 21, 2011 at approximately 4:37 P.M., where two employees were utilizing a 20 lb propane torch to heat shrink anti-corrosion pipe wrapping on 4" steel piping, the employer failed to remove all sources of ignition at the end of a gas piping system in which residual gas was being purged with compressed air. Reference: [NFPA 54 Chapter 7.3.3 (2002 Version): The open end of piping systems being purged shall not discharge into confined spaces or areas where there are sources of ignition unless precautions are taken to perform this operation in a safe manner by ventilation of the space, control of purging rate, and elimination of all hazardous conditions.] Abatement Note: One means of abatement to correct this hazard would be remove all sources of ignition at the discharge end of the gas line to be purged.
