Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)Railroad car spur, on or about 10/26/2010: Two (2) employees were preparing to move a loaded rail car. One employee was standing adjacent to the winch and was struck by the snatch block when it broke loose from the attachment point. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: 1.Ensure three (3) personnel are assigned to perform the car pulling operation safely as required in the Hoosier Magnetics Inc Operations Procedure, Subject: Moving train cars to be loaded, dated 6/10/1998. 2.Ensure that the block (stationary) pulley is attached at an appropriate secure attachment point. 3.Ensure personnel operating the pulling rope is inside the protective cage when tension is applied to the pulling rope in accordance with the Hoosier Magnetics Inc Operating Procedure, Subject: Moving train cars to be loaded, dated 6/10/1998.
