Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being crushed: a)On or about 6/2/10, Madison County Land Fill: Employee was working under or near a raised roll off dumpster tilt frame without the lifting frame safety device in use. Among other methods, feasible and acceptable abatement methods to correct these hazards are to: 1.Train all drivers as to the safe use and operation of their equipment and on the use of the lifting frame safety devices in accordance with ANSI Z245.1 - 1992, section 6.1 a and section 6.2.1 b. 2.Monitor the employees operation in accordance with ANSI Z245.1 - 1992, section 6.1 b. 3.Install signs near the lifting rail support arm indicating the proper use of the safety device in accordance with ANSI Z245.1 - 1992, section 7.1.5. 4.Include the inspection, maintenance and replacement of hydraulic hose abrasion protectors to the companies preventative maintenance and inspection program in accordance with ANSI Z245.1 - 1992, section 5.3. 5.Utilize a visual or audible warning indicating that the tilt frame is raised in accordance with ANSI Z245.1 - 1992, section 7.1.15.1. 6.Label all operating controls in accordance with ANSI Z245.1 - 1992 section 7.1.4.1. Abatement documentation must be submitted for this item.
