Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY: a)Assembly area, on or about 04/27/2010, a Yale 2 ton overhead electric hoist had an upper and lower limit safety switch that did not function in accordance with the manufacturer's manual as well as a broken hook safety latch and no recorded annual inspection had been conducted. Among other methods, several feasible and acceptable abatement methods to correct this hazard are to: 1.Repair or replace the upper Limit switch, lower limit switch, and the hook safety latch and ensure they are adjusted to meet the requirement of the manufacturer's operation manual, Section: Checking limit switch operation and ANSI B30.16.-1993, Overhead Hoists, Section 16.2.3.3 Adjustments, Repairs, and Replacements. 2.Conduct frequent and periodic inspections of the hoists in accordance with ANSI B30.16-1993 Overhead Hoists, Section 16-2.1 Inspection. b)Assembly area, on or about 04/27/2010, the Overhead hoist monorail did not have a rated load capacity marked on the rail: Among other methods, several feasible and acceptable abatement methods to correct these hazards are to: 1.Mark the rated load capacity on the rail ANSI B30.11-1980, Section 11-2.1.2 Rated Load Markings-Monorails. 2.Conduct frequent and periodic inspections of hoist monorail systems in accordance with ANSI B30.11-1973 Monorail Systems and Underhung Cranes, Section 11-2 Inspection, Testing, and Maintenance.
