Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the overhead hoist or bridge/gantry crane fail: a)The following overhead hoists had a broken safety latch and upper limit switch was not functioning: 1.Tool Room, on or about 3/31/10: The upper limit switch on a Duffy Lynx one ton overhead hoist was not functioning. 2.Front Production Dept., on or about 3/31/10: CM Cyclone 3 ton hand hoist on a portable gantry crane at the Stokes press #16 had a broken safety latch. Among other methods, feasible and acceptable abatement methods to correct these hazards are to: 1.Adjust the upper limit switch in accordance with ANSI B30.16-1993, Section 16-2.3.3. 2.Replace the safety latch on the lifting hook in accordance with ANSI B30.16 Section 16-1.2.9. 3.Conduct frequent and periodic inspections of the overhead hoists in accordance with ANSI B30.16-1993, Section 16-2.1. b)The following monorail beam/gantry cranes had no rated load markings on them: 1.Tool Room, on or about 3/31/10: A monorail beam with a Duffy Lynx one ton overhead hoist and a Budgit two ton hand hoist attached, had no rated load capacity markings on it. 2.Front Production Dept., on or about 3/31/10: A portable gantry crane with a 3 ton CM Cyclone hoist and a one ton Budgit hoist attached had no rated load capacity markings on it. Among other methods, one feasible and acceptable abatement method to correct this condition is to: 1.Mark the rated load capacity on the monorail beam and gantry crane in accordance with ANSI B30-11-1980, Section 11-1.1. Abatement certification must be submitted for these items.
