Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by or crushed by hazards: On or about February 22, 2010, and times prior thereto, at 5755 Highway 61 South in Wilson, Arkansas, southeast corner of the plant, the 1959 #1487 Freight Elevator that was being used by employees to transport materials between the 1st and 2nd floor levels was equipped with an approximately 51" high gate that did not extend the approximately 6'8" height of the elevator car. The gate did not prohibit employees from having any part of their body outside the elevator gate while the elevator was in operation. This condition exposed employees to struck by and caught between hazards.
