Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: 1) On or about 11/22/2010 at 1101 East 33rd Street, in Hialeah, Florida, on the shipping/receiving areas the employer did not ensure that an employee operating a Mitsubishi FGC15N powered industrial truck (serial number AF81F20136) fastened his seat belt. The employee was exposed to crushing hazards. 2) On or about 11/22/2010 at 1101 East 33rd Street, in Hialeah, Florida, on the shipping/receiving areas the employer did not ensure that an employee operating a Mitsubishi FGC15N powered industrial truck (serial number AF81F10386) fastened his seat belt. The employee was exposed to crushing hazards. 3) On or about 11/22/2010 at 1101 East 33rd Street, in Hialeah, Florida, on the shipping/receiving areas the employer did not ensure that an employee operating a Mitsubishi powered industrial truck, serial number AF82F10901, fastened his seat belt. The employee was exposed to crushing hazards. ABATEMENT NOTE: Among other methods, one feasible and acceptable method to correct the hazard is to ensure that employees fasten their seat belts when operating powered industrial trucks in accordance with the manufacturer's instructions. PLEASE NOTE: CERTIFICATION OF ABATEMENT IS NOT REQUIRED FOR THIS VIOLATION.
