29 CFR 1910.119(e)(3)(v): The process hazard analysis did not address the hazards of toxic material release in facility siting: a) Jobsite: Plant 17, Marcus Hook Refinery - Employees who were relocated from the old Plant 17 Control Room to a new blast resistant control room and shelter module were potentially exposed to toxic hazards without an evaluation of the impact of toxic material release prior to occupancy. This was observed on or about 4/25/2011. NOTE: ABATEMENT DOCUMENTATION REQUIRED
