Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads and/or lift parts from the improper operations of: (a) Upper Plant, Crane building - The control pendants of four underhung cranes: 7.5 Ton Kona Crane, 10 Ton Stahl Crane, 15 Ton Detroit Crane and a 2 Ton Crane were not legibly marked to indicate the function and direction of movement. Employees from two working shifts used these cranes everyday. Observed on or about 02/11/11. (b) Upper Plant, Crane building - Stops were damaged at the end of the 10 Ton Stahl Crane travel to prevent the crane from inadvertently coming off the track. Employees from two working shifts used this crane everyday. Observed on or about 02/11/11. (c) Upper Plant, Crane building - The cable of the control pendant for the 10 Ton Stahl Crane was damaged. Employees used this crane everyday. Observed on or about 02/11/11. (d) Upper Plant, Crane building - The upper limit devices were not inspected during the functional test inspections for the four operational cranes. The employer conducted daily visual inspections which did not include testing of the upper limit devices. Observed on or about 02/11/11. (e) The employer did not conduct frequent nor periodic inspections of the four operational cranes. Observed on or about 02/11/11. Among other methods, one feasible and acceptable method of abatement would be to follow the requirements of ASME B30.11-2010. NOTE: ABATEMENT DOCUMENTATION REQUIRED.
