Text For Citation: 01 Item/Group: 001 Hazard: GUARDING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the point of operation of a hydraulic press whose operation exposes an employee to amputation injury, was guarded by a light curtain that was not regularly maintained: a)On or about May 19, 2011, Department 24, Modular Assembly, a 355 Mod ULC Cell press used a presence sensing device (PSD), i.e. light curtain that did not conform to consensus standard (ANSI B11.2) and manufacturer's requirement. Discrepancies include, but not limited to: (1) the employer failed to establish a procedure for inspection and maintenance of the light curtain based ont he manufacturer's instructions and recommendations; (2)test using manufacturer's test object (p/n 42800-0110) or an opaque cylindrical test object 0.75 inches in diameter with established test procedures were not performed before the start of each shift or as recommended by the manufacturer; (3)operator training on PSD test procedures, PSD operations,and severe consequences of bypass attempts was not provided before the employee initially operated the press; (4)the PSD was not recertified by the employer and revalidated by an OSHA-recognized third- party validation organization at least annually; and (5)records of the installation certification and validation and the most recent re-certification were not maintained for each PSD equipped press by the employer for as long as the press is in use. Among other methods, one feasible and acceptable method to correct this hazard would be to insure that the light curtain conform to consensus standard (ANSI B11.2) and manufacturer's requirements. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
