Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Facility - On or about 03/17/11 employees operating the tire mounting machines #2, 3, and 4 were exposed to struck by hazards from tire failures. Abatement method: Among other methods, a feasible and acceptable method to correct these hazards is to eliminate employee contact with defective tires by displacing the tires into a conveyor protected by a guard (cage). b) Facility - On or about 03/17/11 employees operating the five manual tire mounting machines were exposed to struck-by hazards from tire failures. Abatement method: Among other methods, a feasible and acceptable method to correct these hazards is to ensure that the rim is firmly secured to the machine and a guard (cage) is installed to prevent the projection of tire pieces during a tire explosion. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
