Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard: a) At the employer's work place in Williamsport, PA: A Clark forklift was operated with the seatbelt removed exposing the operator to a crushing hazard, on or about July 27, 2011. ABATEMENT CERTIFICATION REQUIRED Among other methods, one feasible and acceptable abatement method to correct this hazard is to: a) Follow the instructions on the piece of equipment which states do not remove the factory installed operator restraint system. b) Follow the safety standards published on page 10, section 5.3.19 of ANSI ITSDF B56.1- 2005, Safety Standards for Low and High Lift Trucks which states that an active operator protective device or system, when provided, shall be used. c) Instruct each operator of the requirement to always use the seatbelt when operating a forklift and to immediately notify their supervisor if the seatbelt has been removed.
