Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that personnel were exposed to fall hazards while working from an elevated platform attached to a Toyota powered industrial truck (forklift): (a) Facility, 102 Freedom Drive, Lawrence, PA 15055: On or about January 31, 2011, an outside contractor was moved horizontally and lifted up approximately 20 feet above floor level, in an unsecured wire mesh collapsible container (wire parts basket) by a Hanco employee operating a Toyota forklift. This practice is prohibited as described by the Toyota Operator's and Owners Manual and by ANSI/ITSDF B56.1-2009 Safety Standard for Low Lift and High Lift Trucks, Section(s) 4.17; 4.18; 4.19; 5.1; and 7.37. Among other methods, one feasible and acceptable abatement method to correct this hazard is to utilize an operator-up high lift truck, an aerial lift or a scissor lift.
