Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to severe burns, traumatic injuries, and/or death due to the reaction of water mixing with molten metal: (a) Facility, Casting Department: On or about January 8, 2011, employees were exposed to the rapid expansion of water into steam and the pressure that occurs when molten metal explosions when molten metal reacts with water in the Casting Pit. Among other methods, one feasible and acceptable abatement method to correct this hazard is (1) Prevent molten metal from contacting standing water in accordance with the American National Standard for Safety Requirements for the "Melting and Pouring of Metals in the Metalcasting Industry", ANSI Z241.2. (2) Develop an inspection procedure to ensure there are no cracks in molds; that all stool caps and molds are properly aligned; that bad stool caps are "tagged" out of service until repaired/replaced; and to preheat all molds to ensure that no unsafe moisture conditions exist in them prior to casting.
