Text For Citation: 01 Item/Group: 001 Hazard: CAUGHT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or about February 23, 2010 and times prior thereto, at 9301 Frazier Pike in Little Rock, Arkansas, the employer did not ensure that each employee working in the Coating Final Inspection area were prohibited from entering areas where transfer lifts were being raised and lowered and when pipe transfer activities were taking place, exposing employees to the hazard of entrapment between the lifts and pipe. On or about February 6, 2010, an employee was stenciling inside an approximately 42" diameter 80 ft. pipe. The lift control operator, who was unaware the employee was inside the pipe, lowered the transfer lifts and when doing so the employee's leg was caught between the pipe and lift. As a result, the employee sustained severe leg injuries.
