Text For Citation: 01 Item/Group: 001 Hazard: CONFINED

P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of falling down inside conveyor chute and not being immediately recused: A) Mulcoa conveyor system, on or about May 24, 2011, employees enter into permit required confined space with out proper fall protection equipment, and no retrieval equipment in the immediate vicinity, exposing employees to a fall and entrapment hazard of approximately eight feet and not being able to be rescued quickly. One feasible and acceptable abatement method: Install a ladder that employees can use to reach the area they need to clean and provide a safety harness and retrieval system designed to rescue employees from that environment and assure that the supervisory person is adequately trained on permit required confine space entry and rescue. ABATEMENT CERTIFICATION IS REQUIRED.
