Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by/crushed by a moving elevator car while working on top of the car. a)At the worksite, 242 West 104th Street, New York, NY 10025 On or about 01/13/10. Employees were exposed to being crushed between the top of the elevator car (crosshead) & the top of the hoistway, while working atop the moving elevator car examining car top cables, and a stop switch was not provided on top of the elevator car. Among other methods, one feasible and acceptable method to correct this hazard is: Comply with the American National Standards for Elevators stipulated by the American Society of Mechanical Engineers (ASME) ASME A17.1-2000, "Safety Code For Elevators And Escalators" - Section 2.26.2.8 "Stop Switch on Top of Car" which states that a stop switch conforming to 2.26.2.5(a), (b), and (c) shall be provided on the top of every elevator car. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.
