Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
29 CFR 5(a)(1): of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are exposed to amputation hazards. On the Kill Floor of the facility located at 1631 C Ave Dakota City, NE 68731 - The employer is failing to ensure employees do not enter the area on the backside, "blind side," of the carcass while the split saw is in operation. This violation was most recently observed on the Kill Floor, where employees were entering the blind side area while the split saw was being operated to cut the carcass in half. Employees on the blind side are exposed to being struck, and lacerated by the split saw as it moved through the carcass. One or more feasible methods of abatement include... 1)Ensure employees are adequately trained on split saw operation, hazards of the blind side, and the policy to limit access to the blind side area. 2)Develop, implement and train employees on the company discipline policy outlining the rules forbidding employee entry into the blind side. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.ion
