29 CFR 1910.106(b)(6): Sources of ignition were not eliminated or controlled where the pressence of flammable vapors was possible: a- Hess Port Reading Refinery, Dimersol - On 04/23/10 a frac tank being used during the final water wash and drying of the R-2203 caught fire and ruptured. At the time of the incident, a spill guard was installed under the portable metal atmospheric tank which contained wash water, sludge and hydrocarbon. Adequate precautions were not taken to prevent the ignition of flammable vapors to include eliminating static electricity through proper bonding and grounding.
