Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that the rated load of underhung cranes was not marked on the crane, exposing employees to struck/crush-by hazards: a) Advantage Powder Coating, Inc. - Defiance, Ohio: On or about January 24, 2011, the employer did not assure the grinding department north side underhung crane, used to lift and suspend parts, was labeled with the load capacity. A load test was not conducted and/or load test documentation was not available to determine the load capacity of the "in-house" fabricated underhung crane system. b) Advantage Powder Coating, Inc. - Defiance, Ohio: On or about January 21, 2011, the employer did not assure the grinding department south side underhung crane, used to lift and suspend parts, was labeled with the load capacity. A load test was not conducted and/or load test documentation was not available to determine the load capacity of the "in-house" fabricated underhung crane system. Among other methods, one feasible and acceptable means of abatement is to follow the American Society of Mechanical Engineers (ASME) B30.11-2010: Monorails and Underhung Cranes as applicable. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation ( using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
