Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: a) On June 7, 2010 and at times prior thereto; at the center of the facility, the freight elevator on the second floor was able to be opened without the car/platform being present exposing the operator(s) to a fall hazard of approximately 30 feet to the lower level. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS ITEM NOTE TO EMPLOYER: Feasible means of abatement for this would be: 1) Conduct an inspection by a qualified elevator inspector and schedule periodic inspections according to ASME A17.1 2) Install interlocks to prevent the elevator door from opening when the car is not present according to ASME 17.3 3) Instruct employees not to override safety mechanisms and not to use the elevator if there are defects
