Text For Citation: 01 Item/Group: 001 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards: On or about 09/04/2009, on Rig #11, Concord,OH: Employees working on the derrick platform were exposed to fire and explosion hazards in that an auxiliary escape line was not set up for quick evacuation from the derrick platform in the event of an emergency. Among other methods, one feasible and acceptable method to correct this hazard is to install an auxiliary escape line that uses a specially rigged and securely anchored escape line attached to the derrick or mast, so as to provide a ready and convenient means of escape from the derrickman's working platform, as referred to in Paragraph 6.10.1 of API RP 54, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, 1999 Edition.
