Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to fall hazards: a) On or about June 23, 2010 and times prior employees were exposed to injuries from falls to the lower surface in silo #3 in the following locations; 1) The top floor control switch is located where the elevator travels exposing employees to a fall hazards when employees reach into the elevator shaft to press the call button. 2) When an employee exits the elevator at the lower level the distance from the bottom of the elevator floor to the floor is 19" exposing employees to a fall hazard. Among other methods,feasible and acceptable means of abatement would be to, relocate the call switch for the elevator outside of the elevator shaft, and relocate the limit switch so the elevator stops at a safe level for exiting the elevator.
