Text For Citation: 01 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act: The employer failed to provide employment free from recognized hazards that are causing, or are likely to cause, death or serious physical harm: ANSI Std. B56.6-2005 - Safety Standard for Rough Terrain Forklift Trucks: 8.24 Platforms for Elevating Personnel 8.24.1 (b) floor dimensions...shall not have a width greater than the overall width of the truck (measured across the load bearing tires plus 10 inches on either side.) Employees were working from a basket that was being supported by a SkyTrak rough terrain forklift truck. The basket was 20 ft. by 4 ft. The width of the forklift was 8 ft. 1 in. The basket is made so it can be elevated by the forklift with the forks on the 20 ft. side of the basket or with the forks on the 4 ft. end of the basket. It is used both ways, but most often with the forks on the 20 ft. side. The basket should not exceed 9ft. 9 in. to be used with this forklift. Employees were working at heights of up to 10-1/2 feet. Employees are exposed to the hazard of falls.
