Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards due to defective or damaged hooks and safety latches; (a)On or about December 1, 2010 near the automatic end former, the Dayton 1 ton chain hoist used to raise and move coils of metal tubing weighing up to 500 pounds had a defective safety latch. (b)On or about November 10, 2010 at the clean line, the P&H overhead crane used to raise and move baskets of metal parts had both hooks with a metal bar welded across the throat opening. Reference for hooks and safety latches, ASME B30.10-2009, Section 10-1.10.5(i) and Section 10-1.10.6(c). Welding on hooks, Section 10-1.10.5(l). Inspections for damages to hooks, Section 10-1.10.3. Among others, one feasible and useful abatement method to correct this hazard is to: perform inspections of hooks for damages and to replace hooks that have been welded, and to repair or replace defective safety latches.
