Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by, crushing, and/or amputation hazards: (a) In the large lathe department for employees operating the Dainichi CNC lathe, serial #18101, where the CNC lathe was not equipped with a functioning door interlock device exposing employees to the potential hazard of being struck by parts expelled from the turning lathe or being caught in the moving machinery inside the turning lathe by opening the lathe door during machine operating cycle. (b) In the large lathe department for employees operating the Dainichi CNC lathe, serial #17709, where the CNC lathe was not equipped with a functioning door interlock device exposing employees to the potential hazard of being struck by parts expelled from the turning lathe or being caught in the moving machinery inside the turning lathe by opening the lathe door during machine operating cycle. (c) In the medium lathe department for employees operating the Okuma LC 30 CNC lathe, serial #54080058, where the CNC lathe was not equipped with a functioning door interlock device exposing employees to the potential hazard of being struck by parts expelled from the turning lathe or being caught in the moving machinery inside the turning lathe by opening the lathe door during machine operating cycle. (d) In the medium lathe department for employees operating the Mori Seiki SL-35 CNC lathe, serial #1386, where the CNC lathe was not equipped with a functioning door interlock device exposing employees to the potential hazard of being struck by parts expelled from the turning lathe or being caught in the moving machinery inside the turning lathe by opening the lathe door during machine operating cycle. (e) In the medium lathe department for employees operating the Okuma LC 40 CNC lathe, serial #55120342, where the CNC lathe was not equipped with a functioning door interlock device exposing employees to the potential hazard of being struck by parts expelled from the turning lathe or being caught in the moving machinery inside the turning lathe by opening the lathe door during machine operating cycle. (h) In the medium lathe department for employees operating the Okuma LC 30 CNC lathe, serial #55030146, where the CNC lathe was not equipped with a functioning door interlock device exposing employees to the potential hazard of being struck by parts expelled from the turning lathe or being caught in the moving machinery inside the turning lathe by opening the lathe door during machine operating cycle. (i) In the medium lathe department for employees operating the Okuma LC 40 CNC lathe, serial #55070244, where the CNC lathe was not equipped with a functioning door interlock device exposing employees to the potential hazard of being struck by parts expelled from the turning lathe or being caught in the moving machinery inside the turning lathe by opening the lathe door during machine operating cycle. (j) In the medium lathe department for employees operating the Okumahowa twin spindle CNC lathe, no serial number identified, where the CNC lathe was not equipped with a functioning door interlock device exposing employees to the potential hazard of being struck by parts expelled from the turning lathe or being caught in the moving machinery inside the turning lathe by opening the lathe door during machine operating cycle. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install a door interlock device on the CNC lathes as required by ANSI B11.2-2002 Safety Requirements for Turning Centers Automatic Numerically Controlled Turning Machines, paragraphs 6.21 Safeguarding and 6.21.1.2 Automatic Mode which states: 1. Paragraph 6.21: Machines shall be safeguarded wherever practical by fixed or interlocked moveable guards. 2. Paragraph 6.21.1.2: Machine operation shall only be possible when the guards are closed and the safeguarding (protective) devices are active. If it is possible, in this mode, to open an interlocked moveable guard, the hazardous movement shall cease and be inhibited.
