Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazard injuries: Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to fall hazards from a self propelled and elevating work platform (scissor lift). On or about October 13, 2010 and times prior thereto, at the worksite, employees had not received general training regarding the inspection, application and operation of aerial platforms. Specifically, employees were using the lift to access upper work levels, such as but not limited to bridge cranes. Two employees attempting to troubleshoot and/or repair the hoist of the P & H 10 ton bridge crane (serial number 35242) were using a scissor lift (serial number 10936) as a means to access the hoist control box. The scissor lift, which was positioned between the trolley rails, was struck by the hoist resulting in the lift overturning. This condition exposed employees to a fall of approximately twenty feet. Among other methods, feasible and acceptable means to correct this hazard include, but are not limited to the following: 1. Assure the scissor lift is operated and employees are trained in accordance with the acceptable requirements of the American National Standards Institute (ANSI) for Self- Propelled Elevating Work Platforms, ANSI/SIA A92.6-1999. 2. Assure when aerial platforms are operated within the area of travel of cranes or other moving equipment, precautions are taken to prevent collisions. 3. Assure the operation of crane controllers is fully understood and that all controllers are working in a predictable and appropriate manner.
